Site Survey- Door Supervisor

This form must be completed before security is provided at a venue.

The Security Industry Authority can prosecute any unlicensed person proven to be performing S.I.A licensable activities. This means that for a person to manage or control a Door Supervisor, that person must themself hold a valid SIA Door supervisor license or a SIA Non frontline license.

To deliver the best possible service while complying with the law, this survey is intended to establish the house rules that will be enforced by the Security Operatives deployed to the venue. The strategy and techniques applied by the Door supervisor/s for any security related issue will be entirely at their discretion in line with their SIA conflict resolution and physical intervention training.

If possible after introducing yourself, but before completing the survey with the client, take the time to walk through the venue to gain an understanding and make notes of the immediate vulnerabilities, you observe as this may influence questions you ask during the survey process.

Use the Notes box at the end to include these notes and any other questions asked that aren't included in this survey.
